Unstable angina refers to symptoms that are due to impaired blood flow through the coronary arteries that is inadequate to meet metabolic demands, but not to the degree that actual cell death is occurring. Controversy exists regarding the necessity of rapid reperfusion in NSTEMI, although the American College of Cardiology / American Heart Association (ACC/AHA) guidelines do recommend an early interventional strategy for those patients with evidence of myocardial necrosis, as demonstrated by elevated cardiac biomarkers. Non-ST segment elevation myocardial infarction (NSTEMI) refers to myocardial cell death in the absence of diagnostic criteria for STEMI.
